Rates and Contract for Services
The rate for professional care management services is billed at $75 per hour, and mileage (according to federal rates) may be charged.
The service agreement outlines the legal and financial responsibilities of the client and of Eldercare Advocacy & Management. The Responsible Party, as identified in the service agreement, agrees to make full payment upon receipt of the bill. Billing is on a bi-monthly basis. The agreement may be terminated by the client, the Responsible Party, or Eldercare Advocacy & Management, at any time upon 14 days written notice.
The service agreement must be printed out, signed, and mailed or faxed to Eldercare Advocacy & Management. The agreement cannot be e-mailed.
